The short answer is yes -- if you are waking up with back pain on your mattress that you feel is attributable to your mattress, then you most likely do need a new mattress. That said, the subtext of your question seemed to be that you are surprised to be replacing your mattress after only 5 years, especially since you are lightweight and have been careful about rotating your mattress. On this front, I would simply note that when this type of thing happens, it can be because either your mattress has changed (eg, due to normal wear over time) OR because your body has changed (eg, due to normal aging, body shape changes, changes to your preferred sleep positions, etc.). Sometimes it can be subtle changes in both of these. But either way, if you are waking up with back pain that you feel is related to your mattress, it's likely that it's time for a new mattress.
